SV4 Hope Annual Walk Participation Waiver

By registering for the SV4 Hope Walk, I acknowledge that participation in a walking event involves physical activity and certain risks. I confirm that I am physically able to participate and voluntarily assume all risks associated with the event.

I agree to release and hold harmless the SV4 Hope Foundation, its organizers, volunteers, and sponsors from any claims or liabilities arising from my participation in this event.

If registering additional participants or minors, I confirm that I have the authority to agree to this waiver on their behalf.
